What the inquiry form collects and where it goes.

The public inquiry form collects the information a visitor chooses to provide using these fields: name, email, phone, company, buyer_type, message. A hidden anti-bot field may also be processed.

Current form route

The website submits the form over HTTPS to an endpoint on api.chosencrm.com. Do not use the public form for confidential, regulated, controlled, clinical, financial, or authentication information.

What not to send

Do not submit passwords, one-time codes, PHI, CUI, tax or banking information, payment-card data, social-security numbers, protected solicitation content, or other sensitive records.
